SSM Health St. Mary's does not have copies of birth certificates. To obtain a copy of a birth certificate for an individual who was born in Wisconsin, you must request a copy from the state of Wisconsin Department of Health & Family Services, Office of Vital Records. Receive

How to Obtain Copies of Medical Records
In order to obtain copies of your medical records, you are required to fill out an "Authorization for Information" form. You may download the PDF file of the Authorization for Information form below, or you may call to request this form by calling the Health Information Management Department at 608-373-8060.
We require the patient or legal guardian to complete an authorization prior to mailing copies of records. The original records are kept at SSM Health St. Mary's. You may request copies of your records at any time.
