Phase III: Outpatient Maintenance Program
This exercise program is for Phase II graduates or individuals considered appropriate for Phase III after a physician evaluation. The goal is to provide support and guidance as you move toward an independent exercise program. After 12 visits, a staff member will discuss your progress.
This is a self-pay program that requires physician referral. Participants purchase a punch card for $50. The card is valid for 12 sessions and expires six months from the date of purchase. Participants may exercise up to five days a week. Staff will inform you if an entry assessment and telemetry monitored session is necessary before you begin. If this is required, the cost is $50. Some insurance companies may cover part of the Phase III costs. Contact your insurer for details.
